CASE ORGANIZATION
The Food Stamp Face Sheet will be filed on top of the case record. Social Security Cards and SS-5's will be attached to the back of or filed immediately behind the Food Stamp Face Sheet. A divider sheet will be used on the bottom of the case record. The brad will enter the case from the bottom. The contents of the case record will be maintained in three sections - the Case Summary Section, the Overpayment/Fraud Section, and the Documents Section.
CASE SUMMARY SECTION - This section will be filed immediately behind the Face Sheet. It will contain all case actions including certifications, denials, changes, etc. The data will be filed with the keyed authorization document on top, followed by the action document which initiated the case action, followed by the supporting forms and documentation. The most current case action will be filed to the top of the section.
Certifications and case changes will be filed in the order shown below.
1.Authorization Documentation (EMS-233, EMS-211)
2.Action Document (EMS-220, EMS-234, EMS-237, EMS-271, EMS-85, EMS-244, printed screen of WFAI)
3.Supporting Forms (EMS-1, EMS-260, EMS-206, EMS-205, EMS-1410)
4.Supporting Documentation (copied verification)
5.EMS-239
Second party forms will be filed on top of the case action which was reviewed.
OVERPAYMENT/FRAUD SECTION - This section will be filed following the Case Summary Section. A divider sheet will be used to identify the section. It will contain all claim and fraud documents such as the EMS-216, EMS-217, EMS-1700, etc. with the most recent data filed to the top. Information received or submitted about a specific claim will be filed together. For example, the demand letter for an overpayment will be filed on top of the EMS-216 and EMS-217 related to the letter.
DOCUMENTS SECTION - This section will be filed following the Overpayment/Fraud Section. A divider sheet will be used to identify the section. It will contain all legal documents related to the case, e.g. - divorce decrees, and birth certificates.
