8120The Household's Rights at Application

A household filing an application for food stamps has the following rights.

1.The right to receive, upon request, an application form from the

county office;Requests may be in writing, in person, by telephone, or through another agency or individual.

2.The right to file an incomplete application in the county of residence either in person, or by mail or through the use of an authorized representative. An incomplete food stamp application must contain at least the applicant's name and address and the signature of either a responsible member of the household or the household's authorized representative.

Applications not containing at least this information will not be registered but will be returned to the household if possible.

3.The right to receive assistance in completing the application form if such assistance is requested.

4.The right to file an application during regular office hours on the same day the household initially contacts the county office to request an application form.
